Architectural Application Checklist

For architectural change applications for play equipment,  include the following items:

1. Architectural Change Application.
2. Location of equipment on copy of lot survey.
3. Drawing, sketch, photograph, or brochure showing equipment along with equipment description.
4. Color/finish.
